When paying for treatment with your private medical insurance you will need to ensure you are fully aware of the terms and conditions of your policy, including any limitations which may apply and liaise closely with your insurance provider at each stage of your treatment.


Confirm with your insurance provider that you are covered for your planned treatment and your first assessment appointment.  

Some insurance providers will require:

  • Date when  your symptoms first developed
  • Obtain referral from your GP/ provide a GP signed claim form
  • Insurance policy provider signed claim form

 

Please note

Please check the details of your policy carefully. You may have an excess fee, limits on the amount you can claim on certain types of treatment or some charges may not be covered under your insurance plan. All shortfalls will be payable by the patient, you will be notified of the amount in advance by your insurance company.
