Insured Patients

MY Eye Clinic are registered with AXA PPP Healthcare, AVIVA, Cigna, WPA, Bupa, Saga and Benenden Health.

When paying for treatment with your private medical insurance you will need to ensure you are fully aware of the terms and conditions of your policy, including any limitations which may apply and liaise closely with your insurance provider at each stage of your treatment.

Confirm with your insurance provider that you are covered for your planned treatment and your first assessment appointment.

Some insurance providers will require:

  • Date of when your symptoms first develope
  • Obtain referral from your GP/ provide a GP signed claim form
  • Insurance policy provider signed claim form

Please Note:

Please check the details of your policy carefully. You may have an excess fee, limits on the amount you can claim on certain types of treatment or some charges may not be covered under your insurance plan. All shortfalls will be payable by the patient, you will be notified of the amount in advance by your insurance company.

Obtaining Referral For Treatment

Arrange an appointment with your GP and simply request a referral to a consultant at MY Eye Clinic. Your GP will then arrange a referral letter, either you or your GP practice can contact us to arrange your assessment appointment.

Please note – some GPs may change a nominal fee for completing sections of your claim form (if required by your insurance provider).

Confirming Cover With Your Insurer

You are responsible for the costs of your treatment, please ensure you contact your insurer to confirm your policy covers you for your planned treatment and assessment. It is important you understand the cover provided by your insurer.

We would recommend you contact your insurance company at each stage of your treatment, including your assessment appointment, to confirm your treatment plan and confirm cover.

Information To Bring To Your Appointment

When attending the clinic for your assessment and treatment, please bring along:

  • A completed claim form and/ or proof of pre-authorisation
  • Private medical insurance registration documents including:
    • Registration number
    • Level of cover
    • Scheme details (if insured under a company scheme)

Please bring your policy documentation with you to all your appointments, even if you have attended MY Eye Clinic before.

If you are unable to provide all of your insurance documents, you may be asked to settle your account in full.

Billing and Payment

Invoicing for most insured patients is sent directly to the insurance provider electronically. Should an invoice be sent to an insured patient’s home address, this should be forwarded to your insurer.
